Apply for a Library Card

Your New
Holstein Public Library card may be used at the public libraries in
Manitowoc, Two Rivers, Chilton, Kiel, and Brillion.

When
applying for a library card, you may choose a regular card or a
smaller 'key chain' card at no cost. Replacement cards are
available for $1.00. Or you may choose a deluxe card set that includes
both a regular and key chain card for $2.00.

You will
be asked to provide a standard form of identification. A driver's
license or state I.D. is preferred. Other forms of I.D. are acceptable
under certain conditons.

The
signature of a parent or guardian is required on library cards issued
to anyone under eighteen years of age.

When
using your card to access your library account on the online catalog,
enter the entire 16 digit card number. Your PIN number is the
last 4 digits of your telephone number.

You may
fill out an application form at the library or click in the box below
to print a form that you may fill out now and bring to the library.
